Please read these Terms of Use (“Terms”) carefully before using www.hersheyprimarycare.com (the “Site”), operated by Hershey Primary Care, PLLC (“Hershey Primary Care,” “we,” “us,” or “our”). By accessing or using the Site, you agree to be bound by these Terms. If you do not agree to these Terms, please do not use the Site.
These Terms are separate from, and in addition to, our Website Privacy Policy and HIPAA Notice of Privacy Practices.
1. Medical Disclaimer
The information and materials provided on or accessed through this Site are intended solely for educational and informational purposes and are not intended nor implied to be a substitute for professional medical advice, diagnosis, or treatment. Individuals should always seek the advice of a physician or other qualified health care provider prior to starting any new treatment, selecting a method of treatment, or seeking answers to any questions regarding a medical condition.

2. No Physician-Patient Relationship Through the Site
Submitting information through our contact form, online scheduling tool, listserv sign-up, or general email (hello@HersheyPrimaryCare.com) does not establish a physician-patient relationship and does not constitute a request for medical treatment. A physician-patient relationship is only established once you have become an enrolled patient of the practice through our standard enrollment process.
Our contact form and general email address are not intended for medical questions, urgent concerns, or anything requiring a timely clinical response, and are not HIPAA-compliant communication methods. If you are an established patient and need to reach your physician about a medical matter, please call our office or use the Patient Portal. If you are experiencing a medical emergency, call 911.
We do not guarantee a response time to inquiries submitted through the Site and are under no obligation to respond to or act upon any information submitted through it.
3. Use of the Site
You agree to use the Site only for lawful purposes. You agree not to:
- Use the Site in any way that could damage, disable, overburden, or impair it
- Attempt to gain unauthorized access to any part of the Site, other accounts, or computer systems connected to the Site
- Use any automated system (bots, scrapers, etc.) to access or extract content from the Site, except as permitted by standard search engine indexing
- Submit false, misleading, or fraudulent information through any form on the Site
- Use the Site to transmit any harmful, threatening, abusive, or unlawful content

We may update these Terms from time to time. The “Effective Date” at the top of this page reflects the most recent revision. Continued use of the Site after any changes constitutes your acceptance of the revised Terms.
9. Governing Law
These Terms are governed by the laws of the Commonwealth of Pennsylvania, without regard to its conflict of laws principles. Any disputes arising from these Terms or your use of the Site shall be subject to the exclusive jurisdiction of the state and federal courts located in Pennsylvania.
